Send a referral
For yourself, for a parent, or for a client. We respond within one business day and we will tell you honestly if we are not the right fit.
Referral details
A few details are enough to start. Please leave medical reports and detailed health histories out of this form; we can arrange a suitable way to share them later.

What happens after you send it
No obligation at any stage, and nothing starts until you have a written breakdown in front of you.
- 1
Talk to us
We call you within one business day and ask about the person and the household, not just which services you are funded for.
- 2
Care planning
We set out which supports we would provide, on which days, at what cost, in writing.
- 3
Matching your carer
We match on language, gender preference, cultural fit and availability, then brief the carer before the first visit.
- 4
Services start
Support begins on the agreed schedule with the same carer wherever possible. We check in early, then keep checking in.
Referring a client?
We accept referrals from care partners, support coordinators, hospital discharge teams, GPs, allied health clinicians and community organisations.
Useful things to include if you have them: current classification or funding level, the supports already in place, language spoken at home, whether personal care needs to be gender-matched, and any clinical risks we should know about before a first visit.
What we will tell you
- Whether we can staff the suburb consistently
- Which carer we would match and their availability
- A written cost breakdown against the client's budget
- If we are not the right provider. We would rather say so than take the referral
Hospital discharges
Contact us before the discharge date rather than after it. Matching a carer properly takes a little time, and rushing it produces exactly the mismatch that causes services to fail in week two. This matters most after a stroke or a fall.
Not ready to refer yet?
Model the cost first. The pricing calculator uses our real published rates and the current Support at Home quarterly budgets.
Have questions first?
Our FAQ covers assessment, classifications, contributions, changing providers and what happens if you are not happy with a carer.
