Registered nurse care and allied health delivered in the home. Under Support at Home, clinical supports are funded differently from everyday living services.

  • Registered nurse clinical careWound care, chronic disease monitoring, medication management, clinical oversight of a care plan, and liaison with the GP and specialists. Frequently the first thing needed after a hospital discharge, and the thing that most often prevents a readmission.
  • Allied health assistanceCarrying out a therapy program between clinical visits. Physiotherapy and speech pathology programs only work if the exercises actually happen on the days between appointments, and this is the service that makes that happen.
  • Occupational therapyHome safety assessment, equipment, bathroom and access modifications, and practical strategies for daily tasks. Under Support at Home, equipment and home modifications are funded separately and do not come out of your quarterly budget.
  • PodiatryFoot and lower limb care, including diabetic foot assessment, nail and skin care, footwear advice and orthoses. One of our directors is a practising podiatrist, so this is core work for us rather than something we outsource.
  • PsychologyMental health support for older people, and where appropriate for the family members caring for them. Depression, grief, anxiety about becoming a burden and carer burnout are all treatable and all routinely left untreated.

Why clinical capability matters when you choose a provider

Plenty of aged care providers deliver domestic assistance and personal care well, then refer out for anything clinical. That is workable until something changes quickly, at which point you are coordinating three organisations that do not talk to each other.

Because we deliver clinical, personal and domestic supports ourselves, a change in someone's condition can be picked up by the carer who visits on Tuesday and acted on by the nurse without a new referral, a new assessment and a four-week wait.

Contributions work differently for clinical supports

Clinical supports such as nursing and allied health do not attract a participant contribution under Support at Home. Independence and everyday living services can, depending on your income, assets and when you entered the system. Our FAQ covers this, and our pricing calculator lets you model the cost either way.

From first call to first visit

Four steps, and we do most of the work

You do not need a diagnosis, a plan number or the right words. Tell us what is happening and we will work out the rest with you.

  1. 1Talk to usCall, email or send a referral. We ask about the person, the household and what a good week looks like, not just which services you are funded for.
  2. 2Care planningWe review needs, budget and preferences together, and set out exactly which supports we would provide, on which days, at what cost.
  3. 3Matching your carerWe match a carer on language, gender preference, cultural fit and availability, then brief them properly on your household before the first visit.
  4. 4Services startSupport begins on the agreed schedule with the same carer wherever possible. We check in early, then keep checking in.
